I guess the first thing I'd do is check both ends of the shift mechanism for wear/looseness that might have allowed it to slip out of gear. As many of us have had this issue.., seems the only real permanent fix is to disable the VCM. My advice for added protection is to make the switch to a full synthetic ATF like Valvoline MaxLife (3 separate drain and fills) and add a transmission cooler., Changing every 30K miles is a pretty good rule of thumb for Hondas, as their transmissions seem to be sensitive to old fluid, and the OEM fluid breaks down pretty quickly in hot weather or stop and go driving. Lots of owners here change out 3 qts of transmission fluid every other oil change or so., Keep in mind, those are the install instructions. The 9 speed is a different beast of a transmission over the older 5 & 6 speed transmissions. With either of them the top off/fill procedure was pretty straight forward. Pull the ATF dipstick, check how far it was down and fill to the filled level line. The 9 speed has no dipstick.
